Checks and Proof of Service

You will receive your juror payment either in person at the courthouse or by mail. Your payment will include:

  • Your juror check
  • Proof of service documentation
  • Any information required for tax reporting purposes

On the back of the proof of service form, you will find a QR code linking to our Juror Experience Survey. We encourage you to complete the survey, as your feedback helps us evaluate and improve the jury process.

The survey includes optional demographic questions. Responses are not linked to your identity unless you voluntarily include your name or juror ID within the survey.

We collect this information to better understand juror experiences and to make informed improvements to the overall jury process.

Mental Health Brochure

If you served for the full length of the trial, a brochure on mental health after jury service has been included with your materials.

Jury service—particularly in lengthy or difficult cases—can have a lasting emotional impact. If you, or anyone who has served as a juror, is experiencing stress or mental health concerns, we encourage you to review the brochure for information and available resources.

If you are having difficulty locating a mental health professional, please contact the Jury Commission. We may be able to assist you in identifying a provider.
